What is a Scaphoid Fracture?
A scaphoid fracture is a break in the scaphoid bone, one of the small carpal bones in the wrist. Despite only being a small bone, the scaphoid plays an important role in the complex mechanics of the wrist, facilitating a wide range of motion. Because of its unique position and blood supply, fractures of the scaphoid are known for their tricky diagnosis and sometimes complicated healing process.
Anatomy of the Scaphoid Bone
The scaphoid is a boat-shaped bone located on the thumb side of the wrist. It sits just above the radius, one of the two long bones in the forearm. It can be felt in the ‘anatomical snuffbox’—a small depression in the skin that becomes visible when the thumb is extended.

The most common cause of a scaphoid fracture is a fall onto an outstretched hand (often abbreviated as FOOSH). This mechanism of injury—where the force is transmitted up through the palm and directly into the scaphoid bone—accounts for most scaphoid fractures. Other, less common, causes include direct blows to the wrist, sports injuries, and motor vehicle accidents.
Certain groups are at higher risk, such as athletes in contact sports (football, rugby, hockey), skateboarders, and individuals involved in activities with a heightened risk of falling, like cycling or snowboarding.

Scaphoid fractures can be deceiving. Unlike other bone breaks, the symptoms may be subtle. Common signs include:
- Pain and tenderness at the base of the thumb, particularly in the anatomical snuffbox
- Swelling on the thumb side of the wrist
- Difficulty gripping or pinching objects
- Pain that worsens with wrist or thumb movement
- In some cases, minimal swelling or bruising, which can lead to delayed diagnosis
Because these symptoms can overlap with a simple sprain, scaphoid fractures are sometimes missed, delaying treatment and complicating recovery.
Diagnosis of Scaphoid Fractures
Diagnosing a scaphoid fracture typically starts with a clinical examination.
Standard X-rays of the wrist are usually performed first. However, because the fracture may be subtle or hidden, especially in the early days after injury, up to 30% of scaphoid fractures are not visible on initial X-rays. If suspicion remains, a doctor may recommend:
- Repeat X-rays after 10–14 days
- Magnetic resonance imaging (MRI), which is highly sensitive for early fractures
- Computed tomography (CT) scans, useful in assessing the exact location and displacement of the fracture
Early and accurate diagnosis is crucial, as missed scaphoid fractures can lead to poor healing or long-term complications.
Diagnostic Algorithm:
A simple algorithm for whether to refer for X-Ray for diagnosis is as follows:
- Post traumatic thumb side writ pain
- With anatomical snuff box tenderness
- Scaphoid Tubercle tenderness
- Pain on compression of the thumb towards the wrist
- There is a strong possibility of scaphoid fracture when signs 1 or 2, or any combination of 1, 2 & 3 are positive.
Treatment Options
The approach to treating a scaphoid fracture depends on several factors, including the location of the fracture, whether the bone fragments are displaced, and the patient’s overall health and activity level.
Non-Surgical Treatment
If the fracture is non-displaced and located in the middle (waist) or distal part of the scaphoid, conservative treatment is often effective. This treatment involves:
- Immobilization in a cast (often what’s called a thumb spica cast) for 6 to 12 weeks
- Regular imaging to monitor healing progress
- Gradual return to activity once healing is confirmed
Patience is key, as the scaphoid’s blood supply is limited—especially in its proximal portion—making healing slow.
Surgical Treatment
Surgery may be recommended if the fracture is displaced, is located closer to the forearm, or if the bone is not healing as expected. Surgical options include:
- Internal fixation using screws or pins to hold the bone fragments together
- Bone grafting in cases of delayed or non-union, using bone from elsewhere in the body or a donor
Surgery aims to restore alignment, promote healing, and allow for a faster return to function in some cases.
Complications of Scaphoid Fractures
Because the scaphoid’s blood supply is poor—especially in the proximal third—complications can occur if the fracture doesn’t heal properly. Potential problems include:
- Non-union: The bone fails to heal, resulting in persistent pain and weakness
- Avascular necrosis: The bone tissue dies due to lack of blood supply, a serious complication that can lead to arthritis
- Post-traumatic arthritis: Chronic pain, stiffness, and reduced function in the wrist
Prompt and appropriate treatment is vital to reduce the risk of these complications.
Rehabilitation and Recovery
Rehabilitation is a critical phase in regaining wrist strength, flexibility, and function after a scaphoid fracture. Once the cast is removed, patients are encouraged to follow a tailored rehabilitation program, which may include:
- Gentle range-of-motion exercises
- Progressive strengthening routines for the wrist and hand
- Grip training to restore function for daily activities
- Gradual resumption of sports and demanding activities as guided by a healthcare professional
Recovery time varies, typically ranging from 8 to 12 weeks for non-displaced fractures treated conservatively, and up to several months for more complex or surgically managed fractures.
Preventing Scaphoid Fractures
Not all injuries can be prevented, especially in active or athletic individuals. However, steps can be taken to reduce the risk:
- Wearing wrist guards during high-risk sports or activities
- Strengthening the forearm and wrist muscles to improve joint stability
- Using proper techniques when falling, such as tucking the arms or rolling rather than landing on an outstretched hand
Awareness and caution are the best defences against this common injury.
The scaphoid bone, though small, plays a pivotal role in wrist movement and stability. A fracture of this bone requires careful attention to ensure proper healing and recovery.
